Time-outs occur in client applications. SQL Server by itself will continue happily, but the client application runs out of patience. Your average time-out setting will be 30 or 60 seconds.
So you need to check which calls result in a time-out within the client application. Then you can start investigating what may be less than optimal with the server-side processes.

The execution time for some action on the server must have exceeded the clients time-out threashold, so it may be a good idea to start a trace and check for batches or procedures running longer than, say 30 seconds.
