   How would you setup a table of contents in sql server report service 2005? The report will be exported to PDF and emailed to the user in that format. The table of contents will not be in an electronic format.

   I believe there will be a behaviour change when you try to render in PDF, as it is change of browser.
   I think there is a not direct way to create such TOC, see http://www.u2u.info/Blogs/U2U/Lists/Posts/Post.aspx?ID=10 this one.
