In a lot of post I read "not allways Index Tuning give you the best recomendation..." and all of us agree with that.

Are in the market similar or best tool?.

I am not sure if there is a commercial or free index tuning tool out there in the market. I suppose the decision tree used by the Index Tuning Wizard for recommendations is good enough for most query loads. What is not factored into the input is the frequency of these queries in relation to other DML/DDL operations that may run concurrently and the fact that your tables will grow. The Index Tuning Wizard recommends based on submitted queries, the current table schema. So it may have to be run more than once to reflect the changes in data (selectivity, fragmentation etc).

I think there is lots of room in the market (are you reading this SQL Server vendors?) to improve upon the Index Wizard. I best guess is why we haven't see such a tool is because most people prefer a free tool from Microsoft that does so-so than a costly product that does much better.
