SQL Server databases upgrade - what kind of tool and approach would you use?

The topic isn't small to answer in few lines, whenever you perform a database upgrade from previous version to current version the steps may be same but not the approach. The whole update task is a complex and sometimes prove risky if proper testing hasn't been followed. No doubt that it would cost the time, money &amp; resources as a whole.
